I have problem with inserting «survey»

Greetings,
First time that I had tried to insert a survey on my text page, it went fine. Then, I had modified the same survey. Now, I can't insert it anymore. When I do click on INSERT AN OBJECT, nothing is working. I had look into the archive, then went back to the original survey but nothing. Someone has an idea of what may be the problem ? thx you in advance

What happens if you insert a suvey into a new text page?

My test:
First I inserted a survey into a text page.
Then I've changed the survey, opened the text page again, deleted the survey and inserted the survey again - and it works!
